Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a Home Health Occupational Therapist for the Manchester, TN area. This PRN role focuses on one‑on‑one patient care in home settings, providing evaluation, treatment, and patient education to support independence and quality of life.

You will collaborate with a multidisciplinary team to coordinate care, establish goals, and document progress while maintaining compliance with state regulations and agency policies.

Qualifications

• Graduate of an Occupational Therapy Education program approved by CAPTE.
• Current OT licensure in the state and CPR certification.
• Two years of adult OT experience; home health preferred.
• Strong observation, communication, and problem‑solving skills.

Responsibilities

• Perform OT evaluation and treatment per physician orders, including adaptive equipment.
• Develop measurable goals and therapy plans to maximize independence.
• Maintain communication among treatment team, physician, patient, and family.
• Document services timely for proper billing.
• Supervise OT assistants in patient care per state act.

Responsibilities

As a Home Health Occupational Therapist, You will:

• Perform occupational therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Occupational Therapy Assistants in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Graduate of an Occupational Therapy Education program approved by CAPTE
• Current Occupational Therapist licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire
• Two (2) years of experience in adult occupational therapy, home health experience preferred
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; physical therapy skills per competency checklist
