Uplift HomeCare is seeking a Home Health COTA to provide in-home occupational therapy in Riverview, FL, with flexible PRN scheduling. You will deliver skilled OT services under supervision to adult patients in their homes and coordinate care with the clinical team.

Qualificações

• Active Florida COTA/OTA license required.
• Graduate of an accredited Occupational Therapy Assistant program.
• Current NBCOT certification or eligibility, as applicable.
• Minimum 1 year of home health experience required.
• Strong communication, organization, and documentation skills.
• Ability to work independently while collaborating with the clinical team.
• Reliable transportation and valid driver's license.
• Current auto insurance and AHCA Level II Background Screening.

Responsabilidades

• Provide skilled occupational therapy services under the supervision of a licensed OT.
• Follow individualized treatment plans and physician orders.
• Help patients improve activities of daily living and independence.
• Provide therapeutic activities and exercises per plan of care.
• Educate patients and caregivers on exercises, home safety, and fall prevention.
• Monitor patient progress and report changes to the supervising OT.
