Job summary

Sobe Innovative Rehab in West Palm Beach, FL is seeking a licensed Certified Occupational Therapy Assistant (COTA) to provide skilled services in patients' homes.

In this PRN, home-based role you will deliver interventions under the plan of care, document accurately, communicate with the care team, and educate families to support independence in daily activities.

Qualifications

• Active Florida COTA license required.
• Associate degree from accredited OTA program.
• Experience with geriatric home-based therapy preferred.

Responsibilities

• Provide skilled occupational therapy in patients' homes per plan of care.
• Improve functional independence and safety through therapy interventions.
• Communicate with physicians, families, and care team.
• Maintain accurate, timely documentation.
• Recommend adaptive equipment as needed.
• Educate patients and caregivers on home safety and independence.
