Job summary

Rock Creek Foundation is seeking a Direct Support Professional (PRN) to provide daily engagement, personal care, and community integration for adults with IDD. You will support independence, safety, medication administration, documentation, and responsive caregiving in a flexible, on-site setting.

The role offers a competitive hourly rate of $20.00–$21.00, with PRN as-needed scheduling. Training, growth opportunities, and a collaborative team environment are provided to help you develop

Qualifications

• Must possess a valid driver’s license with no more than 3 points and have access to a vehicle for work purposes.
• Must have proof of car insurance.
• High School Diploma (or equivalent) and a minimum of two (2) years’ experience working with adults diagnosed with varying intellectual disabilities and or developmental disabilities.
• Must be able to work individually as well as within a team. Must be able to multi-task.
• Must be able to work with “high-risk” consumers who have mental illnesses as well as development or intellectual disabilities as well as physical.

Responsibilities

• Provide daily engagement and support to adults with IDD in program activities and community integration.
• Provide direct care to those with intellectual and developmental consumer.
• Foster positive relationships between yourself and consumer.
• Ensure consumer safety and maintain a safe environment.
• Encourage self-help activities.
• Remind and assist consumers with self-administration of medication.
• Perform personal care tasks, including assistance with basic personal hygiene and grooming.
• Accompany consumers to scheduled appointment.
• Plan meals, shopping for groceries, preparing and serving meals to consumers.
• Teach skills for daily living, such as decision-making and problem-solving.
• Observe and document changes in consumers' physical condition or behavior.
• Perform light housekeeping duties, such as laundry, cleaning, and organizing.
• Assist with personal care needs (e.g., toileting, feeding, hygiene, grooming) depending on individual support plans.
• Foster social skills, independence, and meaningful community participation through structured activities.
• Lead and facilitate group activities and individualized skill-building experiences that align with each participant’s interests and goals.
• Support vocational exploration, community outings, hobbies, or leisure activities as part of a meaningful day agenda.
• Help participants pursue employment-related activities (e.g., pre-employment training, job coaching) when part of the program model.
• Maintain accurate documentation including progress notes, incident reports, and daily logs for each participant.
• Report observations on behavior, participation, goals, progress, and any changes in needs.
• Contribute to periodic reviews and updates of individual person-centered plans (PCP) as needed.
• Provide or oversee safe transport to off-site activities, appointments, and community outings when required by the program.
• Ensure participant safety in community settings, monitor behavior, and follow risk management practices.
• Work collaboratively with other staff, supervisors, families, and interdisciplinary teams.
• Promote participant self-determination, choice, and dignity, using person-centered practices.
• Participate in staff meetings, training, and professional development to improve service delivery.
• All other duties as assigned.
