Evergreen Life Services is seeking a Direct Support Professional PRN to support individuals in daily living activities, community integration, and personal care.

YOU will follow individual care plans, encourage independence, and document progress while maintaining a healthy, safe environment across assigned shifts. PRN options offer flexible scheduling (8–28 hrs/week) with pay of $2/hr above base rate.

Qualifications

• Must be able to read, write, and communicate effectively in English.

Responsibilities

• Assist individuals with daily living activities and personal care.
• Support independence and home maintenance tasks.
• Assist with medication and treatment plans as needed.
• Provide transportation in company or personal vehicle as requested.
