Job summary

The Avenues Transitional Care Center in San Francisco, CA is seeking a licensed Certified Occupational Therapist Assistant (COTA) on a PRN basis. This role works closely with licensed OT staff to help residents regain independence and improve daily living skills in a Skilled Nursing setting.

Qualifications

Current, active California COTA license
Graduate of an accredited Occupational Therapy Assistant program
NBCOT exam completion required for OTAs
CPR/BLS certification with AHA
Continuing education compliance with California OT Board
Excellent verbal and written communication skills

Responsibilities

Implement OT treatment plans developed by the supervising OT
Deliver skilled interventions for ADLs, IADLs, neuromuscular re-education, and upper extremity function
Monitor patient performance and report progress to the supervising OT
Assist in data collection to support progress assessments and care plans
Instruct patients and caregivers in home programs and safety of equipment
Assist with transfers, positioning, and use of adaptive devices
Support upper extremity positioning and therapeutic handling
Lift, reposition, and assist patients with/without devices
Transport patients to therapy areas as needed
