Job summary

Alameda Center For Rehabilitation & Healthcare in Perth Amboy, NJ is seeking a dedicated staff member to treat and document care for residents under the supervision of a Physical Therapist, contributing to an interdisciplinary team.

Responsibilities include following physician orders, providing treatments to help residents maintain or regain function, educating residents and caregivers, keeping timely reports, observing infection control, and collaborating with staff to ensure high-quality care.

Responsibilities

• Follow physicians' orders for resident care and directions of the Physical Therapist
• Provide treatment to help residents maintain or regain function within physical abilities
• Provide preventative measures and adaptive equipment to prevent decline
• Educate residents or caregivers to maintain or attain highest function
• Maintain timely and accurate resident reports for regulators and payers
• Observe infection control procedures
• Ensure resident safety under supervision
• Collaborate with residents, staff, physicians and providers
• Follow Residents' Rights policies
• Remain positive and caring and act in residents' best interests
• Perform duties effectively, timely and professionally
• Perform other related duties as directed by supervisor
