Spring Creek Healthcare Center in Perth Amboy is seeking a dedicated Physical Therapist Assistant to support licensed therapists in delivering patient-centered care. You will help design and implement personalized therapy programs to promote independence and well-being.

Reporting to the Physical Therapy Department, you will conduct evaluations, assist with treatment plans, communicate with patients and families, and maintain accurate records.

Qualifications

• Assist licensed Physical Therapists in the development of individualized therapy plans.
• Conduct evaluations, assessments, and re-evaluations of patients to identify strengths, limitations, and goals.
• Maintain accurate records and documentation of patient progress.
