Responsibilities

• Provide education and counseling to patients and families regarding treatments and goals.
• Evaluate patient status to determine safe and appropriate function and care needs.
• Document evaluations, treatments and care coordination per facility policies.
• Independently manage own caseload to maximize care planning effectiveness.
• Comply with applicable laws, regulations, and facility policies.
• Support special projects and activities aligned with company values.
• Take on additional responsibilities as assigned by leadership.
