Job summary

CareCo Medical in Connecticut is hiring Registered Nurses for per diem in New London County. You’ll perform 1-to-1 in-home patient care, with pay for each completed visit and required documentation.

The role emphasizes independent work, compassionate patient interaction, and flexible scheduling to balance life and care responsibilities. Responsibilities include home visits, coordination with multi-disciplinary teams, and patient/family education on home therapy.

Qualifications

• Current license as an RN in Connecticut.
• Valid driver’s license and reliable transportation.
• Willing to travel within New London County.
• Strong communication and interpersonal skills.
• Home health experience; Medicare preferred.

Responsibilities

• Perform home visits for patients in a home care setting.
• Observe, records, and coordinates care with multi‑disciplines regarding the patient's treatment and any change in condition.
• Teaches, supervises, and counsels the patient and family regarding home therapy procedures and other care needs.
