Job summary

Smart Move Physical Therapy in Hamilton, NJ is seeking a licensed Physical Therapist to join our outpatient rehab team on a per-diem basis (20-30 hours/week), offering scheduling flexibility and a diverse caseload.

You will evaluate patients across ages, develop evidence-based treatment plans, and implement manual therapy, therapeutic exercise, neuromuscular re-education, and functional training. Collaboration with physicians and the interdisciplinary team ensures continuity of care and growth.

Qualifications

• DPT or equivalent from an accredited program.
• Active NJ Physical Therapist license or eligibility.

Responsibilities

• Perform comprehensive initial evaluations for patients across ages (pediatric to geriatric).
• Develop individualized, evidence-based treatment plans based on goals and findings.
• Implement interventions including manual therapy, therapeutic exercise, neuromuscular re-education, and functional training.
• Progress patients and modify plans based on clinical response.
• Educate patients and caregivers on injury prevention, home exercise programs, and long-term wellness.
• Maintain timely EMR documentation in compliance with guidelines.
• Collaborate with physicians, case managers, and interdisciplinary team for continuity of care.
• Provide outpatient orthopedic and post-operative care.
• Treat sports injuries and return-to-function programs.
• Address neurological and balance disorders; manage deconditioning and chronic pain.
