Job summary

JAG Physical Therapy in New Jersey is seeking a Per Diem Physical Therapist to support our outpatient team. The role emphasizes assessment, treatment planning, and documentation in an EMR, with an emphasis on patient safety and confidentiality.

You will join a care-first organization with multiple facilities and a strong referral network. A state license and CEU requirements apply; per diem schedule offered.

Qualifications

• Bachelor's or Master's Degree required, Doctorate degree preferred.
• Licensed Physical Therapist in the state practicing in NJ.
• Completed state regulated CEU requirement in NJ/NY.
• Experience/knowledge with an EMR software is preferred.
• Excellent oral, written communication.
• Strong clinical decision-making skills.
• Excellent work ethic and dedication to patient success.

Responsibilities

• Collect and record patient care data following department guidelines.
• Include initial evaluation, daily notes, reassessments, discharge plans and team communication.
• Record daily billing and follow productivity guidelines.
• Prepare for evaluations per department policies and standards.
• Select and administer tests and measures.
• Synthesize evaluation results to formulate diagnosis, goals, prognosis, and plan of care.
• Establish treatment plan; collaborate with interdisciplinary team and patient support system.
• Identify need for consultations and referrals; initiate discharge plan.
• Provide patient education/training.
