Job summary

Journey Hospice is seeking a Per Diem Palliative Nurse Practitioner to provide medical examinations and palliative visits for hospice patients in Atlantic County. This per diem role offers flexible scheduling with no guaranteed hours.

Under supervision with the Hospice Medical Director, you will document visits for Medicare Hospice Benefit eligibility after six months of hospice care and coordinate with the interdisciplinary team to deliver compassionate care.

Qualifications

• Master's-level program designed to educate and prepare nurse practitioners/clinical nurse specialists.
• Pharmacology course of at least 45 hours integrated into the Master’s in Nursing or equivalent.
• National certification requires passing an examination.
• Currently licensed and nationally certified as an Advanced Practice Nurse in New Jersey.

Responsibilities

• Perform patient medical examinations and palliative care visits for Medicare Hospice Benefit eligibility.
• Document visits in patient records and required forms.
• Provide medical care and palliative care interventions for hospice patients as needed.
• Communicate effectively with Medical Director, Administrator and Interdisciplinary Team.
