TheKey Home Health Services is seeking a flexible Licensed Practical Nurse (LPN) to serve home health clients in New Hartford County, CT. Per-diem on-call opportunities with the ability to accept last-minute assignments and maintain individualized care plans.

Responsibilities include direct patient care, wellness visits, medication administration, wound care, and timely documentation. A CT LPN license and CPR/BLS are required, with a valid CT driver's license and reliable vehicle for travel

Qualifications

Active, unencumbered LPN license in Connecticut.
1-2 years clinical nursing experience; home health, geriatrics, or care management preferred.
Current CPR / BLS certification.
Flexible schedule; willing to work per-diem shifts, on-call visits, and accept last-minute assignments.
Valid Connecticut Driver's License, reliable vehicle, and active auto insurance.

Responsabilités

Direct Patient Care: Provide skilled nursing care, routine wellness visits, medication administration, wound care, and health monitoring.
Patient Status Monitoring: Observe, record, and report changes in patient conditions to the supervising RN or physician.
On-Demand & Flexibility: Accept per-diem visit assignments as needed to cover shifts across Fairfield County.
Client & Family Engagement: Build rapport with clients and families, provide health education and support.
Clinical Documentation: Complete and submit accurate clinical notes per policy, state regulations, and HIPAA.
Future Coverage: Transition into extended shift or block coverage as regional demand grows.
