Job summary

TheKey Home Health Services is seeking a Licensed Practical Nurse (LPN) to serve home health clients across New Hartford County with independence and clinical autonomy.

Role involves standard home visits, implementing individualized care plans, routine wellness checks, and direct clinical care on an as-needed basis, with priority given to last-minute assignments across New Hartford County.

Qualifications

• Active, unencumbered Licensed Practical Nurse (LPN) license in Connecticut.
• 1–2 years of clinical nursing experience; home health/geriatrics preferred.
• Current CPR / BLS certification.
• Flexible schedule; willing to work per-diem, on-call visits and last-minute assignments.
• Valid Connecticut Driver's License, reliable vehicle and active auto insurance.

Responsibilities

• Direct Patient Care: skilled nursing, wellness visits, medications, wound care, and monitoring per orders.
• Patient Status Monitoring: observe and report changes to supervisor promptly.
• On-Demand & Flexibility: accept per-diem visits across Fairfield County as needed.
• Client & Family Engagement: build rapport and provide ongoing health education.
• Clinical Documentation: complete timely notes per policy and HIPAA standards.
• Future Coverage: transition to extended shift or block coverage as volume grows.
