Qualifications
- Active California RN license (required).
- Minimum 1 year RN experience (home health preferred).
- Experience in rehab, geriatric care, or community health is a plus.
- Strong clinical, communication, and organizational skills.
- Valid driver's license and reliable transportation.
- Ability to travel throughout Orange County.
Responsibilities
- Provide skilled nursing care in patient homes, including assessments, wound care, medication management, and treatments.
- Develop and implement individualized home health care plans.
- Monitor patient progress and adjust care plans as needed.
- Educate patients and families on disease management, rehab, and senior care.
- Coordinate care with physicians, therapists, and the Director of Nursing.
- Document patient visits in compliance with home health regulations.
