Job summary

Remedy Home Health Care is seeking an experienced Registered Nurse (RN) for per diem home health visits in the West Los Angeles area. Ideal candidates will have a California RN license, 1+ year of RN experience in home health, and the ability to travel within Orange County.

The role offers flexible per diem scheduling and competitive pay per visit, with opportunities to contribute to patient rehabilitation and independent living.

Qualifications

• Active California RN license (required).
• Minimum 1 year RN experience (home health preferred).
• Experience in rehab, geriatric care, or community health is a plus.

Responsibilities

• Provide skilled nursing care in patient homes, including assessments and wound care.
• Develop and implement individualized home health care plans.
• Monitor patient progress and adjust care plans as needed.
• Educate patients and families on disease management, rehab, and senior care.
• Coordinate care with physicians, therapists, and the Director of Nursing.
• Document patient visits in compliance with home health regulations.
