Remedy Home Health Care in West Los Angeles is seeking experienced Registered Nurses (RNs) for per diem home health visits across the West LA area. If you enjoy flexible PRN RN opportunities, mobile nursing, and making a difference in community and senior care, this is the role for you.

As a Home Health RN, you will deliver skilled nursing care in patients’ homes, develop individualized care plans, educate families, and coordinate with physicians and therapies.

Qualifications

• Active California RN license
• Minimum 1 year RN experience (home health experience preferred)
• Experience in rehab, geriatric care, or community health is a plus
• Strong clinical, communication, and organizational skills
• Valid driver’s license and reliable transportation
• Ability to travel throughout Orange County

Responsibilities

• Provide skilled nursing care in patient homes, including assessments, wound care, medication management, and treatments
• Develop and implement individualized home health care plans
• Monitor patient progress and adjust care plans as needed
• Educate patients and families on disease management, rehab, and senior care
• Coordinate care with physicians, therapists, and the Director of Nursing
• Document patient visits in compliance with home health regulations
