Job summary

Visiting Angels is seeking a Home Care Field Registered Nurse (RN)/Case Manager to perform in-home Initial Assessments and ongoing reassessments across Mercer County, with optional Burlington County visits.

This part-time/per diem role offers flexible hours (no set schedule) and most visits occur on weekdays; travel to clients' homes is required, and mileage is reimbursed per company policy. NJ RN license is required and prior home care RN experience is highly preferred.

Qualifications

• RN license in New Jersey required.
• Ideal candidate resides in Mercer County and can travel to Mercer/Burlington counties.
• Experience in home care assessments and strong communication skills are essential.

Responsibilities

• Make appointments with clients in their homes in Mercer County.
• Create initial assessments and care plans based on client needs.
• Conduct follow-up in-home assessments and periodic telephone assessments.
• Revise care plans as necessary and coordinate with CHHAs.
• Supervise CHHAs and conduct orientations/skills assessments.
