Job summary

Visiting Angels is seeking a Home Care Field RN/Case Manager for in-home assessments across Mercer County and optional Burlington County clients. This part-time/per diem role has flexible hours with most visits on weekdays, and no set schedule.

Responsibilities include initial assessments, care planning, CHHA supervision, and ongoing reassessments. Requires NJ RN license, home care experience, and willingness to travel to clients’ homes. Mileage reimbursed.

Qualifications

• RN experience in home care required.
• Excellent verbal and written communication.
• Ability to work independently and manage priorities.

Responsibilities

• Make in-home initial assessments and care plans.
• Supervise CHHAs and conduct orientations.
• Schedule and perform follow-up assessments and interim checks.
• Travel to client homes within Mercer County (and Burlington Counties).
