Job summary

Family of Kidz seeks a Physical Therapist Evaluator (PT) in Ulster County, NY. The role offers part-time or full-time possibilities with compensation around $90/hour for services rendered.

Responsibilities include designing therapies for children, conducting evaluations, and coordinating with school teams and families. Qualified candidates hold a NY PTA license, are passionate about pediatric care, and can manage schedules and data collection.

Qualifications

• Graduate from an accredited university with a degree in Physical Therapy.
• Current New York State Physical Therapy License.
• Bilingual is a plus.

Responsibilities

• Design and implement creative individual and/or group therapeutic interventions for all children on your caseload.
• Administer and write up Physical therapy evaluations.
• Ongoing communication with lead supervisors & coordinators regarding case management.
• Maintain professionalism with families, school teams, and other professionals.
• Maintain accurate and timely attendance and billing logs.
• Create and maintain schedules with assigned families/school personnel.
• Collect data on each IFSP/IEP goal and report on progress quarterly.
• Provide input and attend/teleconference initial CPSE/CSE meetings and/or annual reviews.
