Job summary

Heritage-Valley-Health-System Staunton Clinic is seeking a part-time RN Case Manager to coordinate services for individuals with mental illness and/or intellectual disabilities, supporting recovery and community tenure.

The role provides nursing support to the psychiatric team, including triage and screening for medication concerns. Flex schedule, 24 hours/week, with no weekend shifts.

Qualifications

• Graduate of an accredited school of nursing with current Pennsylvania license.
• BLS certification and PA driver's license.
• Minimum 3 years' experience in a psychiatric setting; experience in case management, UR, or discharge planning.
• Ability to assess and utilize community resources.

Responsibilities

• Coordinate services, education, advocacy, and direct care to individuals with mental illness and their families.
• Provide nursing support to the psychiatric team, triage/screen incoming calls regarding meds/symptoms/patient concerns.
• Travel across clinic locations as needed.
