Job summary

Allegheny Health Network is seeking a Part-Time RN Case Manager for Staunton Clinic. The role involves coordination of services, education and advocacy for individuals with mental illness and their families, with nursing support to the psychiatric team including triage and medication/symptom concerns.

Requirements include Pennsylvania RN license, BLS, PA driver’s license, and at least 3 years in a psychiatric setting with CM/UR/D/C experience.

Qualifications

Graduate of an accredited school of nursing with current Pennsylvania license.
Successful completion of applicable clearances as outlined in HR-106 prior to commencing employment.
BLS certification.
Valid Pennsylvania driver's license.
Minimum of 3 years' experience in a psychiatric setting.
Recent experience in case management, UR, or D/C planning.
Knowledge/ability to assess and utilize community resources.

Responsibilities

Coordination of services, education, advocacy and direct care to individuals with mental illness/intellectual disabilities and their families who require supports to recovery and increase community tenure within the least restrictive setting across the continuum of care.
Provides nursing support to the psychiatric team completing Triage/screening for all incoming calls related to medication/symptoms/patient concerns.
