Job summary

Guardian Angel Senior Services is seeking an experienced Registered Nurse (RN) to join our home care team serving Boston and the Greater Boston area. This part-time or per diem position offers flexible scheduling, mileage reimbursement, and meaningful one‑on‑one client interactions in an independent field-based role with solid office support.

You will conduct in-home assessments, develop care plans, mentor caregivers, and ensure timely documentation.

Qualifications

• Massachusetts RN license required.
• Experience in home care, community health, case management, or visiting nurse is preferred.
• Strong assessment and clinical judgment skills; excellent documentation.

Responsibilities

• Carry out comprehensive in-home client assessments.
• Develop, review, and update individualized care plans.
• Conduct caregiver orientation and quarterly supervisory visits.
• Educate, coach, and support caregivers to ensure quality care.
• Complete documentation within required timeframes and maintain records.
• Monitor client progress and communicate changes to the care team.
• Collaborate with office staff to ensure continuity of care.
• Participate in caregiver education and in-service training.
• Perform routine nursing visits including medication management and vitals within scope of practice.
• Provide hands-on assistance during urgent situations when needed.
