Job summary

Guardian Angel Senior Services is hiring a Registered Nurse (RN) to join our home care team serving Boston and the Greater Boston area. This part-time or per diem role offers flexible scheduling, block options or guaranteed daily hours, and mileage reimbursement while delivering one-on-one client care in home settings.

You will complete in-home assessments, develop care plans, educate and supervise caregivers, and ensure timely documentation.

Qualifications

• Current Massachusetts Registered Nurse (RN) license.
• Home care, community health, case management, or visiting nurse experience preferred.
• Strong assessment and clinical judgment skills.
• Excellent communication and documentation skills.
• Ability to work independently.
• Reliable transportation, valid driver's license, and automobile insurance.

Responsibilities

• Complete comprehensive in-home client assessments.
• Develop, review, and update individualized care plans.
• Conduct caregiver orientation and quarterly supervisory visits.
• Educate, coach, and support caregivers to ensure quality care.
• Complete documentation within required timeframes.
• Monitor client progress and communicate changes to the care team.
• Collaborate with office staff to ensure continuity of care.
• Participate in caregiver education and in-service training.
• Perform routine nursing visits as needed, including medication management, BP checks, catheter changes, and other nursing services within your scope of practice.
• Provide hands-on assistance during urgent situations when needed.
