Job summary

Theoria Medical is seeking a part-time Medical Director and Attending Physician to provide care in local skilled nursing facilities. You will build trust with residents, offering consistent primary care with autonomy over your schedule and support from a collaborative clinical team.

Qualifications

• Active US medical license and board certification.
• MD/DO degree in a relevant specialty (Internal Medicine, Family Medicine, geriatrics).
• Experience as attending physician in skilled nursing facilities preferred.

Responsibilities

• Serve as the attending physician for patients in assigned SNFs.
• Collaborate with NPs/PAs to manage daily patient care and supervision.
• Provide high-quality, patient-centered primary care for acute and chronic conditions.
• Participate in quality initiatives like Chronic Care Management and Remote Patient Monitoring.
