BAYADA Home Health Care is seeking a part-time Licensed Practical Nurse to perform home health visits for our Dartmouth office, serving Wareham, Marion, Mattapoisett and surrounding towns. At least one year of LPN experience is required, with home health preferred. Travel to cases is expected to cover designated territories.

Formación

• A current license as a Licensed Practice Nurse in the state of Massachusetts.
• A minimum of one year of recent experience as an LPN.
• Prior home care experience preferred, but not required.
• Ability to travel to cases as assigned.

Responsabilidades

• Making home visits to clients in designated territories.
• Administration of medication, wound care, treatments, and procedures.
• Monitoring clients' conditions and reporting changes to Clinical Manager and Client Services Manager.
• Following up on doctors' orders and documenting them accurately.
• Performing client assessments as necessary.
• Case management and coordination.
• Document observations, interventions, and evaluations in EMR on a state-of-the-art touch pad tablet.
