Job summary

BAYADA Home Health Care, Inc. is seeking a Part-Time Licensed Practical Nurse (LPN) to perform home health visits in Dartmouth and surrounding towns, including Wareham, Marion, Mattapoisett. A MA LPN license and at least one year of LPN experience are required.

Responsibilities include administration of medication, wound care, client assessments, case management, and documenting observations and orders in electronic medical records on a tablet. Travel to assigned cases is expected.

Qualifications

• Active LPN license in Massachusetts.
• Minimum one year of LPN experience.
• Home care experience preferred, but not required.
• Ability to travel to cases as assigned.

Responsibilities

• Visit clients in designated territories for home health care.
• Administer medications, wound care, and treatments as prescribed.
• Monitor client conditions and report changes to supervisors.
• Follow up on doctors' orders and document interventions.
• Perform client assessments and coordinate case management.
• Document observations and care on EMR via a touchpad tablet.

BAYADA Home Health has an immediate opening for a part-time Licensed Practical Nurse to perform home health visits for our office in Dartmouth. This office provides exceptional home health services to adult and geriatric clients in Wareham, Marion, Mattapoisett and surrounding towns. One year prior clinical experience as a Licensed Practical Nurse is required.

Licensed Practical Nurse (LPN) Benefits:

• Making home visits to clients living in designated territories.
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures.
• Monitoring clients’ conditions; reporting changes to Clinical Manager and Client Services Manager.
• Following up with, executing, and properly documenting doctors’ orders.
• Performing client assessments as necessary.
• Case management and coordination.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a state-of-the‑art touch pad tablet.

Qualifications of a Licensed Practical Nurse (LPN):

• A current license as a Licensed Practice Nurse in the state of Massachusetts
• A minimum of one year of recent experience as an LPN.
• Prior home care experience preferred, but not required.
• Ability to travel to cases as assigned.
