Qualifications
- Current license as Licensed Practical Nurse in Massachusetts.
- Graduation from an accredited nursing program.
Responsibilities
- Make home visits to clients in designated territories.
- Administer medications, wound care, treatments and procedures.
- Monitor client conditions and report changes to management.
- Document doctors' orders and care interventions in EMR.
- Perform client assessments and coordinate care.
