Job summary

BAYADA Home Health Care is seeking a Licensed Practical Nurse (LPN) to perform home health visits in Dartmouth and surrounding towns. The role offers part-time or full-time hours with a flexible day schedule, traveling to clients in Taunton, Raynham and nearby communities.

The LPN will administer medications, provide wound care, monitor patient conditions, document care using electronic medical records on a state-of-the-art touch pad tablet, and coordinate with Clinical/Client Services Managers.

Qualifications

• Current Massachusetts LPN license.
• At least 1 year of recent LPN experience.
• Prior home health experience preferred.
• Ability to travel to cases as assigned.

Responsibilities

• Making home visits to clients in designated territories.
• Medication administration, wound care, treatments.
• Monitoring clients' conditions and reporting changes.
• Following up with, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary.
• Case management and coordination.
• Document observations and interventions in EMR on a touch pad tablet.
