Job summary

Interim HealthCare of the Black Hills is seeking a compassionate Home Health Registered Nurse (RN) Case Manager to join our team. This part-time role serves the Northern Hills communities (Spearfish, Belle Fourche, Lead/Deadwood, Sturgis), providing skilled nursing care and coordinating care plans in patients' homes.

You will conduct assessments, develop care plans, collaborate with physicians, therapists and family members, and educate patients to promote independence.

Qualifications

• Licensed Registered Nurse (RN) in South Dakota.
• Home health experience preferred but not required.
• Strong communication and critical thinking skills.

Responsibilities

• Provide skilled nursing care and coordinate all aspects of patients' care.
• Develop individualized plans of care and manage a caseload from admission through discharge.
• Coordinate care with physicians, therapists, social workers, and home health aides.
• Educate patients and families to promote independence.
• Complete accurate OASIS assessments and clinical documentation.
• Advocate for patients and ensure quality and safety.
• Participate in case conferences and interdisciplinary team meetings.
