Interim HealthCare of the Black Hills is seeking an experienced RN Case Manager to join our Home Health team in Rapid City, SD. This role focuses on developing personalized care plans and coordinating with an interdisciplinary team to support patients at home.

The ideal candidate is a licensed RN in South Dakota, organized, with excellent communication and the ability to work independently. We offer flexible scheduling and opportunities for growth.

Qualifications

• Must be a licensed RN in South Dakota.

• Excellent communicator with strong critical thinking skills.
• Experience with electronic medical records and clinical documentation.
• Willing to transition to home health.

Responsabilités

• Conduct comprehensive nursing assessments and develop individualized plans of care.
• Manage a caseload of home health patients from admission through discharge.
• Coordinate care with physicians, therapists, social workers, home health aides, and other team members.
• Provide skilled nursing visits, education, and disease management.
• Complete accurate OASIS assessments and documentation.
• Educate patients and families to promote independence and confidence in managing their health.
• Monitor progress and adjust care plans as needed.
• Advocate for patients while ensuring safety and quality.

What You\'ll Do

As an RN Case Manager, you'll be trusted to provide skilled nursing care while coordinating all aspects of your patients\' care.

Your responsibilities will include:

• Conduct comprehensive nursing assessments and develop individualized plans of care.
• Manage a caseload of home health patients from admission through discharge.
• Coordinate care with physicians, therapists, social workers, home health aides, and other members of the healthcare team.
• Provide skilled nursing visits, education, and disease management.
• Complete accurate and timely OASIS assessments and clinical documentation.
• Educate patients and families to promote independence and confidence in managing their health.
• Monitor patient progress and adjust care plans as needed.
• Advocate for patients while ensuring the highest standards of quality, safety, and compassion.
• Participate in case conferences and collaborate with the interdisciplinary team to achieve the best possible outcomes.

• A licensed Registered Nurse (RN) in South Dakota.

Home Health experience is preferred but not required. We welcome experienced nurses from hospital, clinic, long-term care, rehabilitation, or other clinical settings who are excited to transition into home health.
