The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients in their homes according to an established care plan. You will help with daily living activities and personal care, following safety protocols and reporting any changes to the supervising nurse.

Responsibilities include bathing, grooming, transfers, vital signs when ordered, and assisting with prescribed exercises. The role requires CPR certification and successful completion of Maxim’s competency program,

Qualifications

• Meets training requirements under state and federal laws where applicable.
• Able to read, follow written instructions and document care given.
• CPR certification must be maintained as required by state rules.
• PPD/chest x-ray if applicable per policy.
• Completion of Maxim screening examinations.
• Participation in Maxim competency program.

Responsabilités

• Assist patients with activities of daily living per plan of care.
• Perform baths, oral hygiene, dressing, toileting, feeding as directed.
• Take and record vital signs when ordered.
• Assist with safe transfers and ambulation.
• Support range of motion and simple therapy activities as ordered.
• Handle patient laundry and maintain safety practices in home.
• Observe and report changes to the nurse supervisor.
• Communicate clearly with clients, families and office.
