Job summary

The Lakes Home Care, Inc. seeks a compassionate Home Health Aide to support patients in their own homes under an established care plan. You will assist with daily living activities and personal care, ensuring comfort, safety, and dignity.

Responsibilities include bathing, grooming, mobility assistance, feeding support, documenting observations, and communicating with the care team. Flexible, part-time hours with a focus on quality of life and independence.

Qualifications

• Meets training requirements per state and federal laws.
• Able to read, follow written instructions, and document care.
• CPR certification current per state regulations.
• Completion of Maxim competency program is required.
• Completion of Maxim screening examinations is required.
• Must meet federal, state, and local requirements.

Responsibilities

• Assist patients with plan of care: baths, oral hygiene, dressing, toileting, feeding.
• Take and record vital signs when ordered (temperature, pulse, respiration, BP).
• Assist with safe transfers and ambulation; support planned exercise.
• Perform ROM and simple procedures per care plan.
• Help with patient laundry and safety with equipment.
• Observe and document patient condition and report changes to nurse supervisor.
• Communicate clearly with client, family/caregiver and office.
