Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to deliver daily living and personal care within patients' homes according to individualized care plans. You will support safe transfers, assist with activities of daily living, and maintain detailed notes on patient conditions.

Responsibilities include measuring vital signs, following plans of care, and ensuring a safe home environment. Part-time schedule with flexible hours and competitive pay are offered to qualified

Qualifications

• Meets state/federal training requirements for home health care.
• Able to read, follow and document care plans.
• Must hold current CPR certification.
• Maintains ability to communicate effectively with clients and family.

Responsibilities

• Assist patients with daily living activities and personal care per care plan.
• Measure and record vital signs as ordered.
• Assist with safe transfers and ambulation.
• Provide range of motion exercises as prescribed.
• Assist with laundry and home safety maintenance.
• Observe and document patient condition and report changes.
