Job summary

The Lakes Home Care, Inc. seeks a compassionate Home Health Aide to assist patients with daily living in their homes per care plan. You will provide personal care, monitor conditions, and report changes to the supervising nurse. Part-time schedule with flexible hours and a focus on quality, compassionate care.

You will follow safety practices, complete required documentation, and collaborate with family and office staff to support independence and well-being of clients in their own homes.

Qualifications

• Meets state/federal training requirements where applicable.
• CPR certification is current and valid.
• Ability to read, follow written instructions and document care.
• Completes Maxim competency program as required.

Responsibilities

• Assist patients with activities of daily living per plan of care.
• Perform baths, hygiene, dressing, toileting and feeding support.
• Monitor vital signs when ordered and report changes.
• Support safe transfers and ambulation as directed.
• Observe safety, perform home safety education, and document observations.
• Communicate clearly with client, family, and supervisor.
