Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients at home according to an established care plan. You will help with daily living activities and personal care, following the specified plan of care to ensure comfort and safety.

Responsibilities include measuring vital signs, assisting with mobility, and documenting patient conditions for the supervising nurse. Must meet state requirements and maintain CPR certification as required.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable
• Ability to read, follow written instructions and document care given
• Possess and maintains current CPR certification in accordance with state regulation
• PPD or chest x-ray if applicable
• Successful completion of Maxim screening examinations
• Participation and completion of Maxim’s competency program

Responsibilities

• Assist patients with activities per plan of care (baths, hygiene, dressing, toileting, feeding)
• Take and record vital signs as ordered
• Safe transfers and ambulation
• Assist with range of motion and extensional therapy
• Handle patient laundry and daily needs
• Observe safety and use equipment properly
• Communicate clearly with client, family, and office
• Observe and report changes to nurse supervisor
