Job summary

The Lakes Home Care Services is seeking a compassionate Home Health Aide to provide daily living and personal care in patients' homes per an established care plan. Responsibilities include assisting with baths, hygiene, dressing, meals, and safe transfers, while following the care plan and communicating with the office.

Requirements include CPR certification, documentation of care, and Maxim competency program completion.

Qualifications

• Meets all federal, state, and local requirements.
• Ability to read, follow written instructions and document care given.
• Current CPR certification in accordance with state regulations.
• PPD or chest X-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation and completion of Maxim’s competency program.

Responsibilities

• Assist patients per plan of care with baths, oral hygiene, dressing, toileting, feeding.
• Take and record oral, rectal, axillary temperatures, pulse, respiration, blood pressure when ordered.
• Assist with safe transfer and ambulation.
• Implement exercise per plan of care.
• Perform ROM and extensional therapy as ordered.
• Help with patient laundry.
• Maintain safety and proper use of equipment.
• Observe and teach home safety practices.
• Recognize emergencies and follow emergency management procedures.
• Follow patient plan of care and complete assigned tasks.
• Communicate clearly with client, family/caregiver and office.
• Observe and report changes in patient condition to nurse supervisor.
