Qualifications

• Active U.S. license and board certification (ANCC/AANPCB/PNCB/AACN for NP; NCCPA for PA)
• Specialties may include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Geriatric Medicine

Responsibilities

• Provide high-quality, patient-centered care to SNF residents
• Conduct daily rounds and address acute or emergent issues
• Prescribe medications and treatment plans
• Communicate with facility nursing staff via phone or secure messaging
• Maintain reliable on-site attendance for scheduled shifts
• Ensure compliance with clinical standards and regulations
