Qualifications

Responsibilities

• Provide comprehensive occupational therapy evaluation based on MD orders.
• Document findings on the evaluation form timely and accurately.
• Develop a treatment plan with long- and short-term goals, frequency, and modalities.
• Obtain MD orders for evaluations, treatments, recertifications and discharges.
