Job summary

Curana Health is seeking a Nurse Practitioner or Physician Assistant to deliver high-quality, proactive primary care to residents in senior living communities and skilled nursing facilities. You’ll practice with autonomy, supported by a team of physicians and care leaders.

This role emphasizes relationship-based care, preventative management, and leveraging advanced technology to improve outcomes and reduce hospitalizations. Travel to assigned facilities is required.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and current board certification in your field.
• Master's Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills.

Responsibilities

• Perform comprehensive health assessments and develop individualized treatment plans for residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic medical conditions with a focus on preventative care and long-term wellness.
• Provide ongoing care management that improves quality of life while helping residents remain healthier and more independent.
• Build Meaningful Patient Relationships with residents, families, and care teams.
• Collaborate Across the Care Continuum with physicians, clinical staff, and facility leadership to coordinate care plans.
