Job summary

Sunbelt Staffing seeks a Nurse Practitioner for a per diem Home Health risk assessment role in Shawnee County, Kansas. You will perform comprehensive assessments in members' homes, about 45 minutes per visit, covering Topeka, Silver Lake and Auburn.

Flexible scheduling is offered with shifts Monday–Sunday and options to tailor weekly commitments. Responsibilities include annual health assessments, reviewing medical histories, educating patients, and identifying gaps in care.

Qualifications

• Active Nurse Practitioner license in Kansas.
• Board Certification through AANP or ANCC.
• MSN from an accredited institution.
• Current CPR (BLS) certification.
• At least 6 months of recent NP experience preferred.

Responsibilities

• Complete annual home health risk assessments for Medicare Advantage plan members.
• Gather and evaluate medical history, existing conditions, and medication usage.
• Educate patients on their medical conditions and medication regimens.
• Identify gaps in care, recommending solutions to enhance patient outcomes.
• No treatment planning or medication prescribing required.
