Qualifications

• Registered by the Commission on Dietetic Registration.
• Certification/License in good standing within your state.
• Knowledge of state survey processes and compliance with nutrition care regulations (preferred).
• Strong organizational and communication skills.
• Maintain a positive attitude and effective communication with building occupants, administration, and HCSG leadership.
• COVID-19 vaccination policies compliance as required by state/local/customers.
• If multi-site RD, valid driver's license required.
• RD-Eligible and provisional licensure accepted in certain locations.

Responsibilities

• Assess residents' clinical nutrition needs and document plans in the EMR.
• Develop, implement, and evaluate individualized care plans based on findings, labs, weight trends, intake, and comorbidities.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to improve residents' health and well-being.
• Ensure adherence to CMS, state, and federal regulations and company standards.
• Provide coverage across multiple buildings as needed.
• All other duties as assigned.
