Job summary

Green County is seeking a Licensed Practical Nurse to perform nursing care within the scope of practice for long-term care residents. Pool staff have flexible schedules and work under the direction of a NCC/RN Supervisor.

Responsibilities include receiving shift reports, conducting rounds, coordinating daily duties, monitoring care tasks, and maintaining accurate resident records in the electronic chart.

Qualifications

• Licensed Practical Nurse certification with 3–5 years of nursing experience, ideally in long-term care.
• Ability to follow the care plan and document care accurately and timely.
• Strong communication and teamwork skills for effective resident care.

Responsibilities

• Receive a report from the nurse on the previous shift and present it to nursing assistants, confirming tasks and deadlines.
• Conduct rounds on the unit, observe conditions, and communicate findings to the relevant staff.
• Coordinate daily duties with the NCC and/or RN Supervisor to implement plans of care.
• Monitor nursing assistants for task completion in line with the care plan.
• Record resident information using electronic charts and the clinical record.
• Provide care and service to residents under supervision and fulfill staff nurse duties.
