Job summary

CLFC Healthcare and Communications is seeking licensed practical nurses for on-site shifts at a Wayne County skilled nursing facility in Lyons, NY. You will manage medication passes and treatments, with an RN supervising on every shift, in a high-performing facility.

You control your schedule with per diem hours, enjoy pay rates $38.00–$42.00 per hour, and receive benefits like tuition assistance up to $1,500/year toward RN, weekly pay, and paid orientation.

Qualifications

• Current, unrestricted New York State LPN license.
• Graduation from an approved practical nursing program.
• Current BLS (CPR).
• One year of recent LPN experience.
• Must pass NYS DOH CHRC and drug screen; immunizations current per NY requirements.

Responsibilities

• Run medication pass and deliver treatments per orders.
• Observe and report changes in resident condition to RN.
• Support and direct CNAs on the unit within NY scope of practice.
• Keep shift documentation current in the facility system.
• Follow infection control, safety, and sign-in practices.
