Job summary

Chapters Health System is hiring an LPN for Continuous Care, working under RN supervision to deliver nursing interventions and patient teaching that maximize comfort and quality of life for patients and families.

Qualified applicants hold an LPN license in the state, with active BLS, and ideally 6 months of LPN experience. Flexible scheduling and infusion course experience are preferred to support care delivery across locations.

Qualifications

• High school diploma or GED.
• LPN licensure in state of employment.
• 6 months experience as LPN preferred.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• For Hope Employees: BLS by Jan 1, 2025.
• For Capital Caring employees: BLS by Nov 1, 2024.
• Mobile Driver - valid driver’s license and automobile insurance per policy.
• Reliable transportation to meet visit schedule.
• Infusion Therapy Course (30 hours) and 8-hour Infusion Class preferred.
• Ability to communicate in English.
• Flexible schedule to meet patient needs.
• Ability to use equipment with visual and auditory cues.
• Ability to handle emotional stress caring for terminally ill patients and families.
• Ability to perform essential functions including lifting and movement as required.

Responsibilities

• Under RN supervision, provides nursing tasks to patients and families such as continuous care, dressing changes, catheter changes.
• Demonstrates clinical skills in medication administration, symptom management, and patient/family relations.
• Observes/monitors patients for changes and reports to RN.
• Reports adverse reactions to RN and physicians.
• Performs venipuncture for blood specimens.
• Site observation and care for CVL lines as needed.
• Documents per policy in medical records.
• May be required to report to work during emergencies to ensure continuity of care.
• Cross-training to Hospice House positions as needed.
