Rehab Without Walls in California is seeking an experienced LCSW to provide counseling within a rehab-focused team. The role supports patients and families across social, emotional, and functional recovery, aligning with measurable goals in a community-based setting.

Qualified candidates hold CA LCSW licensure, a Master's in Social Work, and at least one year in rehab. CPR certification and ability to lift up to 100 pounds are required. Positions in San Diego area travel may be involved.

Qualifications

• Master's degree in Clinical Social Work (LCSW) required.
• CA LCSW licensure required to practice.
• Minimum 1 year of rehab experience post-degree.
• CPR certification is required.

Responsabilités

• Performs comprehensive evaluations of social, emotional needs of patients and families.
• Sets measurable objectives with rehab team collaboration.
• Informs physician and health team about social and emotional factors affecting health.
• Instructs patients and families on coping with social/emotional responses to illness.
• Participates in care coordination and identifies patient needs with the team.
• Directs patient and family to community resources to meet goals.
