Job summary

Source One Management Services, LLC is seeking Nurse Practitioners for flexible in-home Health Risk Assessments (HRAs) for Medicare Advantage members. This PRN role offers autonomy, a minimum eight 7.5-hour days per month, and a streamlined EMR system with a company-provided iPad.

You will travel locally to patients within about 60 minutes of your home, review medical histories, perform basic health evaluations, and document findings.

Qualifications

• Active NP license (excluding Pediatrics, PMHNP and Women’s Health).
• Availability of eight full 7.5-hour days per month.
• Willingness to travel within roughly 60 minutes of home.

Responsibilities

• Complete comprehensive in-home HRAs for Medicare Advantage patients.
• Review medical history, current medications, lifestyle, and social factors.
• Conduct basic physical and mental health evaluations.
• Travel locally to patients in your county and nearby areas.
