Job summary

Source One Management Services, LLC is seeking Nurse Practitioners for flexible in-home Health Risk Assessments (HRAs) for Medicare Advantage members. You’ll create your own schedule with an 8-day monthly minimum, earn competitive pay with incentives, and work with a streamlined EMR on an iPad.

No prescribing, no on-call shifts, and malpractice insurance included. You’ll travel locally to patients within your county and nearby areas, performing HRAs and basic health evaluations in a flexible PRN

Qualifications

• Active NP license required for practice.
• Eligibility to conduct in-home HRAs for Medicare Advantage patients.
• Willingness to travel within the local area and surrounding counties.

Responsibilities

• Complete comprehensive in-home HRAs for Medicare Advantage patients.
• Review medical history, medications, lifestyle and social factors.
• Conduct basic physical and mental health evaluations and document findings.
