Job summary

CareLinx by Sharecare is seeking experienced caregivers to provide non-clinical, in-home care to health plan members and private clients in Poulsbo, WA. Work is on-site in client homes, with part-time/per-diem scheduling managed through the CareLinx app.

Qualifications

• Minimum 6 months caregiving experience, or relevant education/certification (CNA, HHA, MA, or equivalent).
• Access to a smartphone capable of running the CareLinx app (required for scheduling, documentation, and timesheets).
• Ability to communicate in English (bilingual encouraged).
• Reliable transportation to and from client homes.
• Must pass a background check.

Responsibilities

• Assist clients with ADLs: companionship, meal prep, mobility, bathing/grooming/dressing, non-clinical medication reminders, light housekeeping.
• Complete and document care tasks accurately in the CareLinx mobile app during each shift.
• Monitor and report changes in a client’s condition to the CareLinx clinical team.
• Submit timesheets on time through the CareLinx app.
• Communicate with your CarePartner regarding any case updates or client changes.
• Ensure patient privacy, dignity, and well-being in a safe, secure environment.
• Maintain client confidentiality at all times.
